Registration closed for 2011 BC season

To Register for an upcoming Adventure Boot Camp, follow the simple instructions below:

1.  Click here for a printable version of the Registration Forms.  

(If you are unable to open the registration documents in your browser, contact me & forms will be sent to you via email or through the postal service.)

2.  Print & Complete each form clearly & legibly. 
 

3.  Make check or money order payable to Firm Foundations.

4.  Mail forms and Registration fee to:

     Des Moines, Iowa 50315
     ATTN:  Boot Camp

5.  When we receive your registration forms and payment, we will send you an email 
      confirmation along with further boot camp information.
